Routine versus Medical Exams
The doctor performs a routine exam when he checks your vision. These procedures include refractions and are only sometimes covered by your insurance company. Your health insurance covers medical exams which are performed to detect infections and other causes of pain or injury.
**NOTICE** Refraction for Glasses
A refraction is the procedure the doctor does to determine if you need glasses or a change in your glasses. Some insurance plans, including Medicare and some secondary insurance plans, do not pay for a refraction. Therefore, the patient is responsible for this charge.
We can give you a statement for you to send into your insurance company upon request.
A refraction is submitted to your vision plan or your medical insurance if you have routine coverage with the doctor you are seeing. Please check with your insurance company to verify that you are covered with our practice. Contact lens exams are not submitted to your insurance company.
We do NOT accept Davis Vision, Blue Vision, VSP, NVA, Spectera, EyeMed, or Avesis.
HMO Referrals
It is the patient's responsibility to check to see if a referral is needed and to obtain an appropriate referral for his/her visit. If a referral from your PCP is not received at the time of visit, you will need to sign a waiver of insurance coverage before being seen by the doctor. You are also welcome to reschedule your visit for a date after you expect to receive your referral. Referrals are usually required for HMO plans:
